The heart of our hospice family is our team: a group of professionals who provide comfort and care for our
patients and their families in their time of need. The team is comprised of the patient’s physician, Flint River
Hospice’s medical director, registered nurses, licensed practical nurses, clinical social workers, non-
denominational chaplains, home health aids, and trained community volunteers. This team works together with
the patient and their family to develop a personalized plan of care to make the individual’s remaining time as
meaningful and comfortable as possible. This personalized care includes everything from providing expert
medical attention to the patient, to educating family members on how to administer care and monitor comfort, to
helping perform household chores, to offering counseling and bereavement services.


Patient Services

Our top priority is to achieve the highest quality of care. One of the main objectives to reach that priority of
hospice care is the relief of pain and other distressing symptoms to create opportunities for family communica-
tions and closure. The Flint River Hospice team is dedicated to this objective, because you matter to the last
moment of your life.

Each plan of care is customized to fit the needs of the patient and the patient’s families. Flint River Hospice
provides a variety of services in order to meet the family’s needs. The list below features many of those
services; -- the list is not exclusive or inclusive to all of the services that we can offer here at Flint River Hospice.

Specialized services that focus on care not cure by addressing pain relief, symptom management, comfort,
and support for terminally ill patients at home, in nursing homes or assisted living facilities.

24-hour, seven-day availability of our interdisciplinary team

Care provided by a team of hospice-trained professionals including physicians, registered nurses, licensed
practical nurses, clinical social workers, a non-denominational chaplain, home health aides, wound care spe-
cialist, respiratory specialist, DME specialist, pharmacy consultant, dietitian, physical therapists, and special
trained community volunteers.

Educational instruction in care-giving for family and friends to care for their loved one.

Inpatient respite care

Spiritual support for patients, their families, and friends.

Bereavement support for surviving family members, friends, and caregivers for days, months,
year following the passing of the loved one.
Our mission for Flint River Hospice is to take
a total team approach with the patient and
their attending physician. By utilizing this
approach, we are able to enhance a level of
continuity and trust between the patient, family,
and the physician who has been providing care
for the patient over the years. This, too, ensures
a higher quality of hospice care while enhancing
the final months, weeks, or days of life for our
patients and their families.
